Insurance Information

Below is a list of the insurances that we accept or are contracted with. The list may not be comprehensive, and there are some exceptions to the lists below. For further questions, please call the clinic directly, and ask for Joely or April in our Billing Department.

And please be aware that although we try to make sure all patients' insurance issues are handled accurately from our end, payment for services and insurance authorization for procedures are ultimately the individual patient's responsibility. If you are ever unsure about a financial or insurance issure, please call us, as well as calling your insurer directly.

**While your insurance may be listed as one we do accept, please check the list below of specific plans we are not providers for as there are select plans that are network specific.**

The Family Clinic ACCEPTS the following insurances:

-AARP Medicare Complete (through Secure Horizons)

-Aetna

-Blue Cross / Blue Shield (NOT Blue Priority HMO / NOT Mediblue HMO)

-Cigna / Great West Healthcare (NOT providers for HMO Plus, or Silver 5000, Cigna Connect)

-Cofinity

-Coventry / First Health

-GEHA

-Humana (NOT providers for HMO Commercial plan)

-Medicare (If you are already an established patient here. We are NOT accepting new Medicare patients at this time)

-Midlands Choice

-Multiplan / PHCS

-UC Health (Poudre Schools)

-United Healthcare

We are NOT ACCEPTING NEW patients for the following:

-Medicare

-Auto claims

-Workman's Comp

-Medicare fee for service plans (PFFS) or Medicare Advantage Plans

-Immigration physicals

-CICP (Colorado Indigent Care Program)

-Medicaid (name change to Health First)

-Tricare

-Weld County Schools thru Aetna
